Breast enlargement known as breast augmentation, also called augmentation mammaplasty, involves surgical placement of an implant behind each breast to increase its volume and enhance its shape. Often, after weight loss, childbirth, or as a result of aging, the breasts lose volume and their shape changes. Also, many women choose to have their breasts enlarged in order to satisfy their desire for a fuller bustline. Breast augmentation can be performed at any age after the breasts are developed.

Fixed Sized Breast enlargement is customized to each patient's individual preferences. Before surgery, sizing sessions are important in achieving a maximized result. However, further changes in breast size cannot be achieved without further surgery. Currently, all women undergoing breast augmentation receive saline-filled implants which consist of a silicone shell filled with sterile saltwater. One of the advantages of a saline-filled implant is that, because it is filled with saltwater after being inserted, only a small incision is needed. Often an incision of less than one inch is made underneath the breast, just above the crease, where it is usually inconspicuous. Another possible location for the incision is around the lower edge of the areola (the pigmented skin surrounding the nipple). A third alternative is to make a small incision within the armpit. Once the incision is made, the surgeon creates a pocket into which the implant will be inserted. This pocket is made either directly behind the breast tissue or underneath the pectoral muscle which is located between the breast tissue and chest wall.

•  It is often possible to return to work within a week, depending on the nature of your work.

•  No scientific evidence that breast augmentation increases the risk of breast cancer, autoimmune disease, or any systemic illness.

•  No evidence that breast implants affect pregnancy or ability to breast-feed.

Other Considerations:

•  Every surgical procedure carries some risk. Potential complications may include reactions to anesthesia, blood accumulation that may need to be drained surgically, and infection.

•  Changes in nipple or breast sensation may result from breast augmentation surgery, although they are usually temporary.

•  When a breast implant is inserted, a scar capsule forms around it as part of the natural healing process. The capsule may sometimes tighten and compress the implant, causing the breast to feel firmer than normal. If the capsular contracture is severe, it may cause discomfort or changes in the breast's appearance. Additional surgery may be needed to modify or remove scar tissue, or perhaps remove or replace the implant.

•  Breast implants can make performing and reading mammograms technically difficult. Placement of the implant underneath the pectoral muscle may interfere less with mammographic examination.

•  As with other surgical implants, breast implants cannot be expected to last forever. If a saline-filled implant breaks, its contents are harmlessly absorbed by the body, usually within hours.

•  Pregnancy can alter breast size in an unpredictable way and could affect the long-term results of breast augmentation.

•  As of May 2000, Federal regulation prohibits breast augmentation for purely aesthetic surgery in women less than 18 years of age.

What is EXPANSION AUGMENTATION ®?
EXPANSION AUGMENTATION ® is the use of a size adjustable breast implant which can be changed after surgery by increasing or decreasing the volume of the implant by adding or subtracting saline (salt water) through a small remote valve placed under the skin during the primary surgery and adjusted during a postoperative office visit.

Why Choose EXPANSION AUGMENTATION ®?
Expansion Augmentation using adjustable breast implants gives patients more choice since the patient can “wear” the implant and then change the size, feel, and cleavage after the surgery by simple injections in the office examination room.

Most surgeons simply “size” their patients during the operative procedure by adding or subtracting saline after installing the breast implant while still in the operating room. This gives the patient only a “single fitting” for the implant. Later on, as time passes, the implant “settles” and eventually looses volume—most commonly explained as “loss of swelling.” While most patients are happy initially with the size of their implants, eventually they become accustomed to the “new” size and begin to “wish they were larger.”

With EXPANSION AUGMENTATION ® with size adjustable breast implants, the size of the implant can be changed without surgery by adding or taking away saline with a needle introduced into the self-sealing remote valve which is connected to the implant by small tubing—much in the same way the computer is controlled with the “mouse.” The clicker on the mouse is the remote self-sealing valve and the cord from the mouse is the tubing going to the implant which can be controlled by simple injections after the swelling from the surgery goes away. How does EXPANSION AUGMENTATION ® work?

Dr. Fisher, as Chief of the Department of Plastic and Reconstructive Surgery at the United States Navy National Medical Center, originally used “temporary” expander devices to stretch the muscles and skin to develop a “new breast” after patients had mastectomy operations. During this time, Dr. Fisher used the concept of “over-expansion” to achieve a “natural looking” breast. Now, we use the same principle to develop and custom design your new breast and the technique is uniquely ours—more simply stated—our “customized cleavage ™” leaves our patients with” implants without edges.™”
 

Breast augmentation is one of the most common procedures performed by plastic surgeons. In a standard augmentation procedure, the implant size is selected at the preoperative visit. After the surgery, the implant size cannot be changed without another operation. However, with Adjustable Breast Implants, the patient can adjust her implant size after the procedure has been performed. This can be accomplished in minutes without patient discomfort. Some patients will even elect to decrease the size of the implant.

How can the implant size be adjusted?

The Adjustable Breast Implant is essentially identical to the non-adjustable implant except for a few small design advantages. A small injection dome attached to a fill tube is placed under the skin, near the incision. After surgery, the implant size can be increased or decreased by using a syringe to add or remove saline through the injection dome. The patient will see an immediate change in breast size after the adjustment has been performed. The procedure is painless and takes about five minutes to complete. When the patient is happy with her result, the injection domes are simply removed through a small portion of the original incision under local anesthesia.

The Adjustable Breast Implants offer numerous advantages over traditional implants. Many patients will choose the adjustable breast augmentation over the standard technique for one or many of the following reasons:

a.) The ability to achieve a more natural shape to the breasts by temporarily over-expanding the implant and then removing the extra fluid, thereby relaxing the skin. The result, a more natural appearing breast.

b.) The ability to achieve breast symmetry by equalizing the implant size with simple volume adjustments.

c.) The correction of more difficult breast problems such as sagging breasts, tubular-shaped breasts (narrow, not round) or an underdeveloped breast on one side only.

d.) Lower incidence of implant wrinkling which can be seen as wrinkling of the skin. Changing the volume of the implant after surgery can often correct the visible wrinkling.

Adjustable Breast Augmentation can reduce patient anxiety regarding the selection of implant size before surgery by allowing the implant size to be changed after surgery. This gives the patient a unique opportunity to have a direct influence over the final size and shape of her breasts.

Expansion Augmentation using adjustable breast implants allows patients to choose the size, shape and contour of their breast implants after surgery by adding or subtracting saline through a small finger sized remote valve placed under the skin. Custom designed breast cleavage can be realized after surgery to achieve a "natural" breast shape and profile. 
According to the conclusion of a three-year study following women with breast implants, it was shown that a large amount of women returned to plastic surgeons in order to change the implant size.  The advantage of choosing the expansion-augmentation technique is that it will allow the women to more easily alter her implants to a more desirable result. 

Another advantage of expansion-augmentation is that it is able to better adjust breast asymmetries that can commonly exist.  Using expansion-augmentation, volume differences, nipple positions, base widths, and ptosis or pseudoptosis can be more appropriately attuned, which can be difficult to achieve with alternate breast augmentation techniques. 

Since every woman will have individual factors influencing how the breast augmentation will turn out, expansion-augmentation has the ability to allow for a more predictable outcome.  Some women's breast augmentation results will be influenced by their natural shape and personal traits, as well as by the surgical procedure like scarring and other unforeseeable results, expansion-augmentation might be a good option for women that have risk of producing unpredictable outcomes.
